  • Domestic Violence Certificate

  • OSHA Bloodborne Pathogens Certificate

  • HIV/AIDS Certificate

  • CPR Training Certification

  • HIPAA Training Certificate

  • Infection Control Certificate

  • COVID-19 Certificate

  • Prevention of Medical Errors Certificate

  • Elder Abuse, Neglect, and Exploitation Certificate

  • ADRD (Alzheimer’s Disease and Related Disorders) Certification from Elder Affairs

  • Driver’s License

  • Social Security Card 

  • Passport (if not a US Citizen)

  • Car Insurance

  • TB Test Results
